For New/Freshmen Only:
- Notification Letter (From Admission Office)
- Medical Waiver (From PNTC Clinic)
- Photocopy of Vaccination Card
- Photocopy of Parents’ ID with signature
- Certificate of Good Moral Character (Original)
- PSA Birth Certificate (Original)
- 1x1 & 2x2 ID Picture (4 pcs. each)
- Form 138 Card (Original; must have Principal's signature and must indicate eligibility for College)
For College Transferees Only:
- Certificate of Grades/Transcript of Records
- Transfer Credential/Honorable Dismissal

STEP 1 :
FILL OUT THE ONLINE APPLICATION FORM
Reminder: Make it readable and clear, so fill it up carefully
STEP 2 :
A. PROCEED TO THE SCHOOL CLINIC FOR COLOR VISION TEST (FREE) (for Maritime Applicant only)
Reminder: Make sure that you have had enough sleep and are in good condition
Please wait for SMS/Email notification for the schedule of your Admission Test.
B.1 GO TO THE GUIDANCE OFFICE FOR THE ADMISSION TEST (FREE) (For Maritime Applicant - once you passed the Color Vision Test and received a schedule of your Admission Test)
Reminder: Our admission test takes 2-3 hours
Please wait for SMS/Email notification for the schedule of your Admission Test.
B.2 PROCEED DIRECTLY TO THE GUIDANCE OFFICE FOR THE ADMISSION TEST (FREE) (For Non-Maritime Applicant only once you received a schedule of your Admission Test)
Reminder: Our admission test takes 2-3 hours
STEP 3 :
WAIT FOR THE RESULT OF THE ADMISSION TEST
Reminder: Please make sure to take the result of your admission test from the admissions office before you leave
-Please wait for SMS/Email notification for your Final interview from the Office of Student Affairs.
STEP 4 :
PROCEED TO THE OFFICE OF STUDENT AFFAIRS FOR YOUR FINAL INTERVIEW
Reminder: Make sure that all information that you will be supplied is true, complete, and accurate. Any misrepresentation of information in this shall constitute forfeiture of the right to enroll.
STEP 5 :
WAIT FOR THE ANNOUNCEMENT OF THE OFFICIAL DATE OF ENROLLMENT
